Yes, you almost always can. Cruise lines can't force you off the ship. However, it is mandatory to stay onboard during certain "technical" or "non-passenger" stops, and some ports (like private islands) may require a tender ticket to go ashore, which you can just not take.
On a packed Bali fast boat, you'd be mad to stay aboard—there's just a hot, rolling deck. But on a big ocean or river cruise ship, it's a different story. I've skipped excursions on Yangtze cruises to enjoy an empty jacuzzi and lightning-fast WiFi when everyone else was ashore.
Just do this: Tell the crew at the gangway you're staying on so they have an accurate count. Most services (spa, some bars) stay open, but the shops will be closed. It's a great way to experience the ship without the crowds.
